Message letter can be a reaction to a received letter containing an approval or refusal of the request. In this case, it is appropriate to make reference to the previous correspondence or telephone conversation that took place:

If the letter contains the refusal of the request, it has three parts: in the first part you repeat the request to which the answer is given; in the second part you explain the reasons of the refusal, in the third part contains the wording of the refusal.
